We have an excellent guide for Configurable List here: How to Set Up the Configurable List Widget
It will show you how to set up the configurable list so that all of the fileds that you need are there, and how to set them up.
Now, once you do, they will be in one row / line so they will seem very wide, but we can help you make it appear in 2 or 3 lines instead with the CSS.
My suggestion is to setup the fields to be in format that is best for you and to hold all the details that you need and let us know once you do and we would be happy to assist with the CSS.
To do this, you can clone your form to make addition on it. You can see here how to make a clone of your form: How to clone an existing form from your account?
Of course, if you have any issues setting up the fields do let us know and we would be happy to assist with that as well.
