How can we setup Configurable List on our form?

right now i have 2 forms next day single and next day multiple...

i would like to have just one form next day single but with the option to have it have the multiples if needed. I tried to make a Configurable List but i dont know how with all the parts of the form that i need can you assist me please?

It will show you how to set up the configurable list so that all of the fileds that you need are there, and how to set them up.

Now, once you do, they will be in one row / line so they will seem very wide, but we can help you make it appear in 2 or 3 lines instead with the CSS.

My suggestion is to setup the fields to be in format that is best for you and to hold all the details that you need and let us know once you do and we would be happy to assist with the CSS.
