I tested this a couple days ago and it just worked, but I just received a form submission and it did not populate my Salesforce. Can you tell me if everything looks ok on this end?Page URL:

From what I understand you are having an issue with your form integration for salesforce not working properly.
I took a look at your provided page url but unfortunately it seems the page is not found on my end.
Additionally, I traced your email address above using "email@example.com" and found you have a guest account which has this form here http://www.jotform.us/form/52671937611156 on it.
Is that the form that you are having the issue on?
Your account appears to be okay from what I can see though unfortunately I can't inspect the form because you are using a Guest Account and you would need to register for a free jotform account in order for us to check your form on your account properly.
Also, you might want to check and make sure you have your fields mapped properly. I would suggest that you make sure you have your form integrated properly.
