You account was automatically suspended by our phishing detector. We do not allow for the collection of username and/or passwords through our forms. Your "Website Refresh Intake Form" asks for both:
The fields you have that ask for social media links may also cause your account to be suspended.
I removed those fields from your form and reactivated your account. If you have more forms that ask for the same information, I would recommend removing the fields from those forms as well so that your account does not get suspended again.
