I have followed the the instructions on how to make my form responsive.
It seem to have worked a treat apart from 2 fields.
The responses to my 'Group Type' and 'Choose your date' options still hang out on the right!

Please add Mobile Responsive widget to your form. That will make mentioned fields responsive as well.
I have cloned your form and added mobile responsive widget in my cloned form. My responsive form looks like this.

It works well when i just view the form as a link
However is seems to loose its responsiveness when its embeded on my website...
What can I do about this?

It seems like the website itself is not responsive, you can see that when you viewed it on a mobile device, it shows the layout same in the desktop view.
Because the form is set to be mobile responsive, it tries to expand its width the same with the mobile device's screen display.
I also see that you are using different ways to make your form mobile responsive. Currently, there's two ways:
Method 1: You can enable mobile responsive options in the Preferences
And in the Form Designer Tool
Method 2: Using the mobile responsive widget.
If you wan to display the form the same way how it looks like in the desktop view, then you might need to disable all the mobile responsiveness of that form.
If you want to make your website itself mobile responsive, to make the form mobile responsive also, you can try adding/configuring a viewport as mentioned in this guide from Google: https://developers.google.com/speed/docs/insights/ConfigureViewport?hl=en. You just need to add the meta tag mentioned there inside the <head> tags. I'm not sure if this is possible in your end, especially if you are using a website builder.
