I've checked your account and seems you are using a guest account with no forms in it. Is the form associated with any other account? We request you to take a look at the following guide that can help you in adding the form to your facebook page.

the question wasnt answered- and the thread WAS JUST MOVED FROM ANOTHER SPOT BY ADMIN - talk about incompetant - cant answer a question just keep moving it then close the question down eh
