Are you having troubles viewing the form in editor? The form seems to be fine on my side.

Note, that form is divided by Page Break sections when viewed online.

Your second form, 43166033046144, has Conditional Logic set on it that is hiding most fields. You can access your conditions by going to Setup & Embed > Conditions:

The conditions are set as such that the rest of the form will be displayed to you only when Is separation regular employee or subcontractor? is set as either Employee or Subcontractor.

However, the conditions are set to hide this field where you would be making that selection as well. That is why the form appears mostly blank for you.

You can edit your conditions so that they are not hiding this field for you. In order to edit a condition, please click on its pencil shaped icon:

When the condition opens for editing, and this is important, please hold the CTRL key while clicking on each of the top three fields to remove them from the condition that is hiding them:

If everything was done correctly, the rest of the list will remain selected in the condition, while the top 3 rules will no longer be selected:

Now you can click Save to save the changed to this condition, and then do the same on the second condition as well. Click on its pencil-shaped icon to edit it, hold CTRL key while clicking on the top 3 fields in the second rule, and then Save the second rule as well.

Your form will end up working like this one:

Please let us know how it goes.
