Are you referring on the field names that appears on the right side bar of the notification wizard? Or are you referring to something else? Sorry but I am not quite sure which part of notifications you are referring. It seems your last edited form has a working notification.

no i was actually referring to the 3 fields that appear in the top left of the email notification settings. You know where you get to choose the sender for the email and the reply address. I have to verify may email address to use which i did but still the very first field doesn't give me any option to chose name for some reason.Heres another problem we have with the contact forms that are displaying in our website. We get blank hint fields all the time. If you open this in any apple computer or device, iphone and ipad 90% of teh time the field hints will not display?? Its like the device is still waiting for the form fields to load. What is happening with this and how do we fix is because we cant launch our new site until this one thing is fixed?
See image here on the pay per click page, see teh field hints missing? This happens on all the other pages in the site also.
Please advise how i can fix?

In regards to the Sender Name.
Please do note that this field offers you fields on your form, such as Full Name and TextBox fields. Now the issue here is that you do not have any labels, so the fields are not shown to you in there - otherwise the labels text would be.
Now, based on the screenshot I believe that this is the form in question: http://www.jotform.co/form/52735301550851 but this applies to all forms.
When there is no label, the setup is based on the order of the form fields instead of making the selection as you would usually.
So you can either select the top option, or (what I suggest), to create a label, then set it up as you wish and remove afterwards if you want to do so.
I would add and leave the labels, but would hide them with CSS so that the records are labeled correctly as well.

Please do note that I have removed emails from being shown on the image and that the "empty" selection under Sender name is simply because I have clicked on that first gray row - representing the first TextBox field on your form.
