On my Trial Application Form 51914358490965, I have three input fields for, Close Date, Start Date End Date,
I have been informed the since 2015-10-22 theses dates no longer appear on our submissions. I checked the notifier to be sure the fields were correctly done and they are. I checked the code to be sure it had the right id for the inputs and it is OK. then I went to INBOX and none of the forms after that date even show the field names.
I then went to MORE, VIEW SUBMISSIONS, and found no dates captured after October 22.
I was using the Birthday Widget to capture this info and it worked well for a while.
Can you look into this for me.

I just checked and most of the widgets are not there now. I used the birthday widget for this form and it no longer exist.
I have edited my forms with separate input drop fields for day, month, year, for each of the required dates.
I'm no longer using the widget.

We have an issue with our widget a while ago, but everything is good now. They are now back in the forms.
As for the missing data in the widget, did you just hide the fields? Because these fields are currently hidden in the form. These fields won't appear in the live form if you have them set to hide and there's no way for someone to fill it. Your form is working fine in your website so I think this is the cause. But not unless you just hide it after finding it's not working in the form.
Also, are you referring to Birthday widgets at the middle of the form or at the bottom? They are identical so I wanted to know.

Since I posted this I have hidden the fields that didn't work. I will remove them shortly.
I no longer have the widget on the form and have removed them from the French one also. My new method is work fine so that is how it will stay.
I only wish I had been informed by our users there was a problem long before now.
Interesting you say they are working now, I went to widgets and can only list a small number of them and the Birthday widget is not there???

If you go to the widgets menu and type "birth" it should be there
If you still have problems to see it, simply make a fresh login and clear the browser's cache if possible

All is good now. I see all the widgets.
Question about this birthday widget, or widgets in general.
If you are having a problem with a widget will that prevent it from functioning on my form?
If yes, my forms for the most part are embedded at my site using the script. Would they also be effected by a problem at your end?

This recent issue with the widgets is related to its database and now fully restored. It should not affect the widgets on the live form (stand-alone and embedded forms) except when you access its wizard on the form builder which isn't available during that time. However, this has now been prevented from occurring again.
