    I am trying to create a form which the customer will have to agree to the terms by initialing or checking a box for each section of the terms.

    I am attaching an image of the handout we used before with each section that the customer had to initial.  Because of the nature of creating custom team uniforms is not everyday normal life for people, we have to try to set expectations and very clearly spell out how the whole process works and what they can and cant do.

    You can accomplish that easily by adding a terms and conditions widget, we have a couple of them you might want to inspect and use, here is a guide with step by step instructions:

    To create sections, simply add a page breaker

     

    Then set the terms widget as required.

     

    That way the user won't be able to move forward unless the checkbox in the terms is ticked.

    I don’t think I am totally understanding or I may not be clear enough in what I was hoping for. In the pic below, there is multiple sections which explain different parts of our uniform ordering process and I would ideally like to have people either check an agree to box or initial next to each section instead of just one huge list with one agree to button at the end. I want to have them agree to each section because we believe this will help them be more willing or motivated to read each section as opposed to just clicking the agree to button once and being done with the whole terms and conditions page. Hopefully this makes sense, I would love to talk to someone on the phone about it (so much easier than emailing back and forth). My cell # is 918-638-8875.

    You can insert text information along with the textbox fields where they can enter their signature or initials. (FYI, this does not support checkbox)
