Sorry for inconvenience this may caused you. I've cloned and tested your form. I am not able to reproduced same issue that you are having. The form was submitted successfully on my side.
Did you embed your form to your website?
If you did, could you please indicate the URL of your page? This will help us better analyze the problem.

needed to ADD a submit button to the form. I thought it would just be
