Can get user input value within Purchase Order?

We sell large number decals to be put on sails.  The numbers come in various sizes and colors.  The customer wants to order specific numbers (e.g. the 4 digit number that was assigned to their boat when they bought it.)  On my Gear Order page where I sell various accessories within a single Purchase Order, I'm having trouble finding a way to support the purchase of one set of 4 number decals in a selected color where the customer enters the 4 digit number he needs.  In other words, I can create a product of a set of four 10 inch numbers and allow the customer to select the color of the numbers, but I can't provide an input field for the customer to enter the 4 digit sail number he wants.

Can you help?
