I've created notifier emails based on conditional logic.
See the attached images.
When I test the form, ALL of the notifier emails are sent out, not just the ones based on conditions.
I go back to JotForm and all the notifier email conditions have disappeared.
I'm going out of my mind... I've been trying to fix this for two days!

Apologies for the frustrations caused by this behavior.
I have tried recreating the issue with a cloned copy of your form but I was unsuccessful form My account.
I will forward this issue to our developer for further investigation.

Sorry for the delays. Unfortunately, there is no updated response yet from our developer assigned.
I have also checked your jotform http://www.jotform.us/form/52761343779162 for the issue, but so far I could not reproduced the disappearing conditions.
What I did noted was, there were condition rules that have deleted field on them. This ccould be causing problem on the condition script.
I recommend you clean/clear them also and re-save the form.
After cleaning the form, please observe if new incoming submissions still have the issue or none...
Let us know if issue persist.
Our developer assign on the reported ticket will respond here as soon as they news/updates.
