The radio button options show up horizontally instead of vertical when the form is embedded

The preview shows the options on radio buttons vertically (what I want)  but on the site it shows them horizontally.

Since the form view is doing what exactly the form would look like, the issue could be on your website page. If you can make it live and share us the URL, that would be great. We can inspect the page and let you know what needs to be done.

For example, your webpage container is way too narrow than the width of your form. But please try setting up your field label manually instead of the current "Auto". Try setting it up to "Top":
