How to get entire form in the email notification?

Hi! I want to receive my entire form with all the required fields filled in when i get the notifier email. not sure what i am doing wrong. the other form i made works, but this one doesn't.

THe screen shot shows what i receive via email. I just don't want the answers, i would like the whole form.

Looking at your form Randi it seems that all fields are included in the email screenshot.

Now, I presume that you are actually looking to have the same data shown to you as it is shown on your form?

If that is the case, then please do note that you can change your email notification in any manner that you like, and have the same sent to you.

Alternatively, you can send yourself the PDF of the form, which is set to look in the format that you are after.

To send it, all you would need to do is to open the email notification that is sent to you and make sure that the option for the PDF to be sent is checked.
