i don't think you are following my question.
when a client submits their completed form and i receive it in an email i used to see the whole form with the answers but now i just get blank boxes. please see the screen shot of the pdf that i receive. my other form is working properly but this one is not for some reason.

I see Randi that you have opened 3 threads so far in regards to the same issue.
Please in future only open a single form and leave as much details on that single form. Also, please do note that you had left the same details across the same threads, suggesting that the issue is about the email notification, while this is the first thread that mentions PDF - which seems to be the actual issue here.
