Not sure why my version of Adobe Acrobat would make a difference. I have Adobe Acrobat CC and I'm on a Mac. It's not a deal breaker, it's just odd that the PDFs are not exactly the same. Incidentally, testing this weekend, when I had not messed with the PDF design, I was able to actually see the images. There were tiny as the screen cap and the names overlapped like in the screencap. Now it seems, the images are not even showing up. Just very weird.

Oops, and yes. I downloaded the PDF and I can indeed see the images just like your screencap. So again, not sure what the dealio is.

Actually no, the issue has not been resolved. I downloaded the PDF from Jelvin's PDF link as he asked me to check if I could see the images. His PDF from dropbox, I can see the images as in the screencap he provided but my PDF's that are being downloaded into dropbox are as the screencap in my first message.

So now no images are no longer showing up in the PDF.
I looked at some of the PDFs that were created last night when I was testing image download into a dropbox folder (another support ticket) and it looks like those PDFs were actually just fine. Well mostly.
I don't have all the test file times but here's the deal:
9:38, 9:48, 9:53, 10:12, 10:27
the time in bold is showing a PDF with images as shown in my initial screencap above.That being said, the first two times 9:38, 9:48 the images showed up in the folder named:
Artist Event Info (with user defined on the dropdown choices)The time at 9:53, the folder name:
_Artist Event InfoThe times at 10:12 & 10:27 the images showed up in a folder named:
JotForm with the other two drop menus at default.The PDF was send to the above Jotform folder. Here is the screencap.
Image should be after the $12-25 and before the "None of the Above".

Also, I'm not sure what has happened but now both emails, the auto responder and notification, are not showing the image links. I am guessing it might be related to something that was done or is now a whole other separate ticket. Ugh. :(

Please access your form submissions page: https://www.jotform.com/help/269-How-to-view-Submissions and select the submission with file uploads in it. Now, click the "Get PDF" button to download the PDF file of the submission.
Let us know if you were able to view the images in the PDF.
Second, please share us your PDF from Dropbox, the PDF from your original reply. We will download the file and check if the images would show up in the PDF.
Third, your latest (3rd) test in the form doesn't have any file uploads in it. Isn't this the PDF that you're getting? Of course, since you didn't upload any image on this submission, you won't see any in the PDF file as well as in your notification email.
You may consider testing it again with the current folder settings. I checked it and you are using "Jotform" as the folder name and default for the other.

That is super strange because uploaded images. The field is required so I cannot complete the form if I do not upload images. It does not explain the fact that at the 9:53 time which was in the middle of tests that worked (PDF showing images), did not showing images.
I know of the place in "View my submissions" that you are referring to, that you have included in your message. That is not what I'm talking about. I am talking specifically about the PDF in dropbox. Again, let me test and see what happens. It just feels like a never ending strings of challenges.
