First Three Fields Only Being Submitted

    Hi my Clone of Responsive layout form I have submitted twice via my windows phone and while form is complete, only

    Full Name joe
    E-mail joe@joe.com

    this above is getting forwarded to my email.  All fields are complete, form is saved, but only first three fields are being forwarded.  Ideas?

     

    Actually just submitted via my Windows 7 Desktop as well, same exact problem - first three fields only submitted.  Thanks.

    Hi,

    This is because the email notification template only has those three fields.

    You need to edit the notification by following this quick steps:

    1.Under "Emails" click the notification you want to edit.

    2. In the "Compose Email" window, you'll see the email template, these are the three fields you were referring, is that correct? You can edit it an add the other form fields by clicking the "Form Fields" panel at the right side.

     

    You can also try deleting that notification and add a new one by following this guide: http://www.jotform.com/help/25-Setting-Up-Email-Notifications. When you create a new notification, all the form fields will be included in your email template automatically.
