My embedded form is not working anymore.

I did check the form you have embedded in your webpage and found that it is currently disabled. It seems you have set the "Expiration Date" of your form and as the date have already reached, your form was disabled automatically. Please check the screenshot below:

I would suggest you to remove the expiration date from your form and enable it. The following guide should help you:
