Table from which rows can be selected?

I would like some suggestions on how to build a tool that can help me create a customized list of recommendations. Each item would also need to be customized.

Specifically, I want to be able to select a supplement item. Each item would be associated with dosing instructions, source, and expected benefit.

For example, an Item might be Brand-x Fish Oil. For the fish oil, I would want to be able to enter the dosing instructions, such as "Take four pills daily". Ideally, there is a default already pre-populated. Additional "static" information would be "where to buy". Sometimes, there might be more than one source, so perhaps a drop down where different URLs could be entered to help choose where to get the fish oil from. Perhaps maybe even a multi-select drop down. Finally, the benefits  of that particular supplement should be associated with that supplement in a column.

This seems like it should be pretty straightfoward, but I can't figure out how.

 If I understood your question correctly, you want to add conditions on specific fields on the Configurable List widget

Unfortunately, it is not possible with JotForm for now.

However you can use the Multiple Selection Widget instead of Configurable List Widget.
