Form Error: This form is disabled

    hey my users keep getting FORM DISABLED messages but my form is on.

     

    Is there something i am missing? Thank you.

    The reason why the error is shown is because the form is disabled, and it is disabled because you have requested credit card details on the same - which is against our terms of use:

    Phishing You agree that Company may terminate your JotForm Service immediately if a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately

    As such, I have removed the same fields from your form and have reactivated your account, but if you recreate the same fields on any other form, please do note that we can remove the entire form instead.

    I do suggest checking our terms of use to see what can be done or not to be sure that your account and all of its forms get disabled again.
