You can do this using our Dynamic Dropdown widget: http://widgets.jotform.com/widget/dynamic_dropdowns. Here's how to add a widget in your form:

You can also do this using a dropdown field and then conditional logic. First, you need to add the Make field. This will be your first dropdown field. Then add more dropdowns for each of the Model. Second, add a conditional logic to show the model based on the selection from "Make" dropdown:

    Thanks
