I have tested the form, once with success but I then altered the form and it won't allow the form to be submitted.
Mark, it seems that multiple submissions are disabled on your form.
To disable form limits, you can go to Setup & Embed > Preferences > Form Limits, and change Unique Submission to No Check. Currently it is set as a Loose Check, which will prevent you from making another submission with the same browser if you have already submitted that form.
