Require a W-9 form for our school to get a paid plan

    I work for a public school district and require a w-9 and

    a sole source letter to purchase your product
     
    Would like to purchase the Silver Package.

    We have different payment processors handling the subscriptions and depending on the payment processor, we can provide you with w-9 form. If you can let us know the plan that you want to subscribe to so that we can provide you with the link to purchase along with w-9 form.

    The request for sole source letter is moved to a separate thread and shall be addressed shortly.

    You can forward any documentation that needs to be filled out by us to "support@jotform.com" and we will be happy to fill out what we can. 

    If you need to pay via purchase order,  purchase Order is possible with Jotform. However,  the minimum payment is $100 as manual handling is involved. So, the first option for you would be to buy the Premium bi-yearly subscription

     

    Upon choosing any of the links at the bottom, make sure you choose "Purchase Order" at the Method of Payment field. Fill everything in that page, including your JotForm Login Name (which would be your JotForm  Username, not the email address)

    Once you are done, it will redirect you to another page where you will actually see the Address where you must send the check for payment. This is our Payment Gateway's, Plimus. You can hit on "Back" in order to correct anything, then you can print it. Also check the terms at the bottom of the page.

    Make sure to SAVE the Reference Number and/or Quote Numbers and include it when sending the payment. That is important so our payment processor will be able to determine that the payment was made for your account.

     

    Also, standard 15 days processing - excluding non-working days.

    I've used the link above and instead of Plimus, Inc it still says BlueSnap, Inc. Is this correct?

    Plimus is Bluesnap, same company.  My apologies, the image in my post is outdated.
