Hi,
I just got an email noting that my "free" account is near the 100 forms submission. My assistant (who is out on maternity leave) noted that she paid for this program in May or June of this year. I am investigating to make sure this is correct as she didn't send me any information after she paid for the subscription. Is there a way for you to investigate this as well or direct me to someone who can help?

Yes, there is empty 'kwhitehe' account which was upgraded in May, 2015. We have now transferred the JotForm Premium Yearly subscription from 'kwhitehe' to your 'slapke' account.
