Since you are paying your subscription through the payment gateway, Plimus, you can just instruct us to cancel your subscription for you or you can do this do this by finding your Plimus username. Your username is a long string of numbers from the original email that you have received when you paid for JotForm.
Then to reset your password, click on the link below and enter the email address that you used when you paid for your subscription. https://secure.plimus.com/jsp/forgot_password.jsp??designId=1
Once you have logged in, you can simply change billing information or cancel your subscription with us.
