I have a free account that allows 100 submissions a month, as far as i can tell my form stopped forwarding submissions to my e-mail address around the middle of September, i missed 3 leads during this time costing me business, can you let me know what happened as i cant afford for this to happen again as i am only a very small business.

Can you please confirm that this was the form http://www.jotformeu.com/form/43023851325346 in question.
I checked the existing submission data of the form, and I noted that it doesn't have existing submissions for the month of Septermber 2015.
user guide: -How-to-view-Submissions
Since there was no submission on the mentioned date, there will be no submission email at the time also.
Please clarify if this is not correct

I can confirm that is the form, I'm sorry for the mistake, it was August
i am missing submissions from not September.

You are right, notifications failed to be sent on August 13 and 18, I checked your email notification, and I see it's not properly setup:
Sender Name :Make sure you assign it to a field from your form where the user enters her name (usually the"Name" field -or similar) to prevent bouncing issues
In addition to this, you may white-list our domain names, if the issue happens again, as we use Amazon SES method for "noreply@jotform.com" sender.
