I've been messing around with this form a lot to get it to function properly in my website.
However: now it seems to be offline or something. Even the direct link doesn't work:

I just checked your form URL and I see that it is working as expected.
However, I noticed that the "Unique Question" in the "Form Limits" is setup. Is this correct? If not, could you please change it to "No Check" then save the changes, after that, open it again and select the appropriate field. Please note that this will prevent submission if the value selected in the Unique Question already exist in the submissions list.

It is set up as "No Check", because for all questions participants are allowed to reenter the same answers.
In chrome the form seems to be working: Firefox is down though.

Firstly, sorry for the inconvenience. We were aware of the problem and our developers were working to fix it. According to our tests, the problem should be fixed now.
Please, try to preview your forms again and give us feedback about its last state.
