email address bounce backs

This is the form (http://www.jotform.com/52883982851975) you're working with right?
Are you saying that you are not receiving email notifications?
I checked your email and it is not in the bounce list. I did a test submission and I saw that it was sent properly as per the mail logs.

I checked the notification setup and I saw that the "Sender Email" is blank. Please set it to "email@example.com" or "firstname.lastname@example.org". If the sender email is blank, your mail server will detect the message as a spam.
