Show Headers and Text in Submissions

I need to see the entire form with the submission not just the answers they've given. I need the header, blank questions they've skipped even when they're not required... everything that's on the form need to show up in each submission. How do I make that happen?

To show the blank questions (no answers), uncheck the option Auto Hide Empty Fields.
