    How to include a form in a form

    I want, by asking a question Yes/No, open a separate/second form in the existing form.

    That second form can also opened single/normally!

    The only way you would be able to do that is to upload the first forms source code onto your webserver, then with a little CSS/Javascript show/hide your child forms. 

    The Javascript/CSS would probably be relatiavely trivial and we should be able to provide that, but you need to commit to downloading the 'parent' forms source code, as described here and then upload it to your webserver.
