We are having problems with skipped information and text too close to the
top margin of pages when we convert our Jotform to a PDF. This particular
problem comes from this file:

Could you please take a look at this and let us know how to fix it?

Have you tried just adding some spacing to the top of the form before getting the PDF.
You can find a spacer widget on the widgets section of the form builder:

I tried this but the spacer does not appear to be making a difference in
the document. We continue to get text cut off at the top and weird breaks
in the document. I was able to do a "print document" that had better
spacing, but it does not include the fillable fields. Here is what the
version using spacers looks like when I download it as a PDF (vs. printing
as a PDF) to retain the fillable fields.

Our apologies. The widget won't help because the PDF creator will just discard that. Any widget in the form won't be available in the fillable PDF.
I have now forwarded this thread to our developers so they can look into this further. If there are updates, you should be notified through this thread.
For now, using break tag in the Free Text HTML field will work. For example:
