Yes, that was my original clone that I have thankfully been able to clone again and then work on, however twice, forms that I have cloned from the above and then worked on, after hitting inconsistencies and then attempting to revert to older revisions, have lost their Conditions - see below:
Logged out and back in.
Both "broken" forms above - still no Conditions visible.

I have replicated the issue you are reporting. I will escalate this issue to our development team for them o further check the issue. We will inform you on this thread any updates on the issue.
