How can I embed a photo into a form?

I have a standard form and would like to know how I can do the following.

Take a photo of a business card and embed it into my form, I then complete the form with my relevant fields. Once submitted refresh the form so I can start the process again. We would also need to see the photo of the business card on the reports.

I use iPads for the data entry.
