Upload field not showing in the submissions

The Client document uploads are also not showing on lots.

I did check your form's submission page and found that the uploads are being captured correctly. Please check the screenshot below:

 

Yes few of your submissions does not have uploads but it seems your users have not uploaded any files while submitting form. Please be noted that both the file upload form field you have in your form is not marked as required field. Please mark the file upload field as required field and that should solve your problem. Please check the screenshot below:
