The last form submission I received was on 10/19 @ 11:02. The 5 that were submitted after that I have not received (including one submitted a minute after the last one. Maybe its a spam issue? Do you have a list of IP addresses that I can put into my whitelist. Are others having this issue presently? any help would be great, thanks.

There are several failed messages in your account email history. As far as I understand, you have removed your email address from our bounce list, so that the email delivery issue is solved.
