Create matrix table, where the cells are drop down box.thanks for your kind attention.

I'm not sure if the form and form builder is enough for a complete multi level authorization system that you are looking for.
For GPS checking location, you can check the available widgets here: http://widgets.jotform.com/category/mapping.
For the multilevel authorization, you can try setting up conditions to your form. Here's a guide that you can refer to: http://www.jotform.com/help/57-Smart-Forms-Using-Conditional-Logic. You can use conditions to send the email notifications to different personnel. You can then use the Edit Link to be part of the multilevel authorization process, let's say the director needs to approve the submission, he/she will need to use the edit link to update the submission.
