Why did my forms move from separate folders?

I have all of our forms separated by department, why recently have they been dumped out of the folders and back into the general repository?

I started to move some of them back, but would like to know why this happened? We have some users that manage their own folders and I was receiving calls asking why they were moved on the 20th?

We had a database update and probably this situation was a result of it. Please accept our apologies for the inconvenience of this situation.

I checked the account and only 8 forms are outside folders. Is this situation solved by now?
