FORM IS DOWN!!! Help

    My form is not showing the  majority of the information. It worked find yesterday. PLEASE ADVISE

     

    It should look like the screenshot, but it looks like the link does. HELP

    Also, I tried to copy the form and I am getting this..

    Hi,

    It looks like one of your conditions is hiding the majority of your form:

    I also removed some of the CSS added by your theme to restore the background color.  If this is still not the way the form should look, let us know and we will be happy to help.
