is there a way that I can make changes to the form on your site and have it reflect the changes on my site? Do I have to embed the code in my page every time I want to make a change?

I see Sue that you have embedded your code using the source code embed option. This means that, yes, you will need to update the code each time you make a change in order for them to be shown.
If you want the change to be instant I can only recommend using the iframe embed code instead, or the default embed code - which would then be updated as soon as you do a refresh on the page.
