To my understanding it seems that you want to cancel your jotform subscription plan.
I see you paid with paypal so to ensure that this is cancelled properly please refer to this guide here https://www.jotform.com/help/336-How-can-I-cancel-my-PayPal-subscription for further instructions.
Though, it seems that you may have possibly already done this as your account shows that you are marked as cancelled and scheduled to downgrade on 10/27/15.
