Integration with box folder happens sometimes, not other times??

I saw that you integrated your form with Box. Can you please check if yours username and password are correct? If they are not good you can not pass authentication. To check if your Integration is setup good please read this articles:

Is there any specific time frame when you did not get submissions? 

You can also try to re-integrate Box.
