when customers are filling out my form they are pressing submit and it is being held on the "please wait" screen. some customers are able to complete the form and it is email to me. about 50% of the time customers tell me after it submits the get a message saying "thank you for submitting" or something along those lines.
I have also noticed that on iphone only 3/4 of the text shows up so customers can not se the full form

Could you please indicate if you are still having issues? I have just cloned your form and submitted a couple of times. All my submission went through correctly.
I have also tried a couple of iphone emulators and could not replicate the issue your users are having with the mobile device.

Yes it still happens to some people. It shows this on they screen after putting allllll the info in right and then when they go back nothing is on the screen. And it keeps happening

I would recommend you to remove your embed code, since it may be conflicting with the Jquery libraries in your page:
So, get a fresh Iframe code to embed it in your page: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
Before you embed the Iframe code, paste it in a text editor like WordPad, you will notice two part of codes, only copy and embed the Iframe part in your webpage, example:
