I was able to see the issue as well when I checked your jotform http://www.jotformeu.com/form/50203682750348
The form is blank/empty. When I checked on your website, it also shows the issue
I check your account forms, and I found that the form was on your My Forms Archived folder.
Did you archived the form?
You will need to unarchive it also to make the form work properly on the website using its URL.
You can use the Revision History tool as well to restore the form to the previous working version.

I could not determine yet the caused why the current version shows blank form. But using the Revision history to revert to the previous version should restore the form to its working state.
