How to include logo image in the submission PDF?

Just wndering how to make changes to the pdf confirmation that is emailed when a form is submitted.

It does not currently have our logo included the way the access form does.

Please be noted that by default headers, images and text fields are not added to submission PDF. If you want to them to be displayed, you will have to enable "Show Headers and Texts" option in the form submissions page settings.
