Our colleague Carina have a found a glitch on our Import Data App that prevents it from working properly on your form.
A ticket had already been escalated to our next level support and currently, one of our developer was already assigned to fix the issue.
We will notify you here once we have update on the status.
On another note, I see that your data source was an Excel file which fails when importing the data to the form. The Import Data App can also use CSV formatted data as an alternative data source.
If there is a way for you to convert the Excel data to CSV format data, you can also try importing the data using the Import Data app using the CSV data source.
Let us see if this alternative can work as well.

Is there a way for you to share to us an access to the CSV file/data? You can upload it to a Google drive or Dropbox account and share to us a download link.
We can also try importing it to your form.
Since there was a confirm glitch already and our developer assigned will have to take a look on it, I am looking for other ways to help you import the data as well at this time. So please forgive my persistence.

Thank you for sharing us your workaround. We appreciate it. However, to my understanding, you added the CSV file data manually, is that correct? I tried to check if that would work on my end, but the submission data were not imported to my form submissions.
