email received says undefined

Besco Tubular Application form. The form is tallying the correct information however when the people receive the email it only has "undefined". Can't really figure this one out?

I found the similar issues a few pages into the support. It shows removing the undefined in the email field however you have to add something there you cannot save. So the fields on the site look like this.. what will the customer do with this? Do i have to format? its a mess

Hi,

The text that you have provided in the variables used in the email Notification that will be replaced with the data the user has inputted. For example, {locationPreferred} will be replaced with the value selected by the user when you receive the email. You can format it as desired or you can try to create a new Notification Email (by default you will have the table with the fields in your form)?
