My form disappear

    Hi and good day,

     

    Im having problem with my form. All items in my form disappear. Below the link of the form:

    Hello,

    Upon checking, I found one form on your account and it seems just fine. Are you still having the trouble with your form items? If you specify the problem that you experienced, we may assist you better. I am able to see both your form and the form items like the picture below, is this your whole form?

    Hi,

    Now i get my form and its submissions back. But little bit confuse here.

    At MyForm page, submission show only one. but in submission page, there are 6 submissions. 

    Can i know y?
