Adding multiple input fields onto on line

Hello, 

I need to add to my form a set up like below, however, I am having a difficult time figuring out to do so.

Add Spouse: FIELDS   Gender        Date of Birth     Tobacco Use

Add Dependent: FIELDS   Gender   Date of Birth     Tobacco Use
