So the name of my form is correct (RLM November 2015 Training), but when people go to click on it - it pulls up a new window or tab that says a different name (RLM September 2015 Training). That was the name of a form I used last month so I think I got it to say that, but I'm not sure how to get it changed.
You may change the page title from the preference settings. Press the PREFERENCE button
In the GENERAL SETTINGS you will see the option PAGE TITLE, you just nee to modify this text to the title you would like
