    How do I setup conditions with a mix of fields?

    Can I do this with a mix of fields? For instance, my first question is "what is your name" (text field). Then I need to ask female / male ( 1 or 2).

    Do you mean to display the field to ask the gender only when name field is filled? Please see the demo form below if that works for you.

    Hi, I need it to function like the attached. This is the form I have built so far in JotForms with a configurable list, but from what I have been told so far, that will not work. 

    Please see attachment. For example "Gender" I need the selection options to be 1- Male, 2 - Female. "Race" - 1 - African American, 2- American Indian, etc.. But when I get the results, to only show a number. 

    Hope that makes sense. Any help at all would be appreciated. :)

    You need to edit it, click on the wizard icon and replace the values, see animated image example bellow:

    Hi there. Yes, I understand that I need to change in there. BUT I need the RESULTS to show a number, not the actual text. The user sees male or female. But when I get the form, I only want to see 1 or 2.

    Unfortunately, it is not possible to assign/show a number based on the selection made from the fields in configurable list widget. However, you may achieve this by using individual fields and assign calculation values to the fields. Then the values may be inserted to addtional/duplicate field by using conditions so that the field can display only the calculation value.

    Hi, that may work.. But now, another thing. I cannot have the fields on individual lines. I need the form to look like the attached image. Is that possible? 

    Also, just want to make sure that you understand what I am asking.. The user sees male or female. When they submit the form, the results I see are either 1 or 2..

    In that case you can customize your submissions view to display only the fields that contains the numerical values, so uncheck the ones with the text values:

    So, when you download the Excel file, it shows that way:

    That is perfect. I think that is just what I need.

    Okay, so now how do I layout the form the way I need to? Can it all be done just with CSS? The form needs to have rows and columns, some columns will be a dropdown option, some will just be text.
